What Is Venous Insufficiency?

Veins are responsible for pumping oxygen-stripped blood back up towards the heart after it has been circulated through the body and to the vital organs. Sending the blood back to the heart often requires pumping it against the force of gravity. In order to handle this task, veins utilize a system of one-way valves to keep blood flowing in only one direction and take advantage of the contraction of surrounding muscle tissue to help move the blood upward.

As the body ages and vein walls begin to weaken, these valves can stretch and malfunction, allowing blood to flow downward. Blood then becomes trapped in the legs and forces the veins to expand, causing them to swell up into the unsightly varicose veins with which so many people are unfortunately familiar. Trapped blood can also darken veins located near the surface of the skin, creating dark clusters of spider veins. Varicose and spider veins are an inevitability for most people, even with a healthy lifestyle and plenty of exercise.

While vein stripping, a painful surgical procedure that removes problem veins, used to be the primary option for treating venous insufficiency, minimally invasive treatments have become the prevailing method for eliminating problem veins.

Treatment Options

Laser Sclerotherapy

Laser sclerotherapy is a minimally invasive vein procedure that can eliminate both spider and reticular veins. This procedure is completed in our clinic treatment room. During treatment, a laser is aimed at the vein in the skin and fired. The veins then are welded shut and collapse, allowing the body to safely and naturally redirect blood flow to deeper, healthier veins.

The entire procedure is relatively painless and takes a very short amount of time. Large areas can be treated with minimal effort. No anesthesia is necessary, although topical anesthesia can be provided upon request to make the treatment more comfortable. Laser sclerotherapy does not carry the risk of blood clot formation in the major veins as is the case with all sclerosants (liquid detergents), and requires one to two treatments spaced one to two weeks apart for optimal results.

Endovenous Laser Ablation

Endovenous laser ablation (EVLA) is a minimally invasive varicose vein treatment that uses laser energy to collapse and eliminate problem veins. After making a small puncture wound in the leg, a catheter is inserted through the skin to penetrate the targeted vein. Ultrasound technology helps Dr. Fisher guide the catheter into place and ensures a precise and effective treatment. Once the catheter is in place, a thin laser fiber is inserted into the vein to emit thermal energy to the vein walls. The heat supplied by the laser fiber causes the interior structure of the vein to collapse, allowing the body to absorb the damaged vein and safely redirect blood flow.

EVLA is performed under local anesthesia and provides immediate results in just one session.

RF Ablation

This procedure uses radiofrequency (RF) energy to gently heat and collapse varicose veins, allowing the body to naturally redirect the blood to nonvisible veins and gradually reabsorb the treated veins.

Topical or local anesthesia will be administered during the procedure to ensure patients are as comfortable as possible while receiving treatment, although vein ablation is a relatively painless procedure. A puncture wound is made in the leg to allow the insertion of a catheter into the targeted vein.

Dr. Fisher uses ultrasound technology to help guide the catheter safely into the vein and administer the treatment with great precision. Once the catheter has been set into place, an electrode emits RF energy to heat the surrounding vein, causing the vessel to shrink and collapse. Vein ablation closes the defective vein, preventing blood flow from passing through it.

Clarivein

ClariVein is a minimally invasive deep vein and varicose vein treatment that combines mechanical action with sclerosant infusion to safely collapse and eliminate problem veins. Unlike other minimally invasive vein treatments, ClariVein does not require numbing or the use of anesthesia, further lowering the risk of complications.

A small puncture wound is created to allow insertion of a catheter into the targeted vein. Ultrasound guidance allows for accurate and precise treatment. The catheter is equipped with a rotating wire tip that spins gently but rapidly inside the vein, stimulating the vein to spasm and to cause vein contraction. The injected sclerosant causes the cells of the veins inner most lumen to die and slough off, ultimately causing vein collapse. Once the vein has been treated, Dr. Fisher will remove the catheter.
